Schedule for Booster Shots and First-dose Vaccine at the Ateneo Grade School Cafeteria - QC LGU Vaccination Site - 26 JANUARY 2022

Subject: Schedule for Booster Shots and First-dose Vaccine at the Ateneo Grade School Cafeteria - QC LGU Vaccination Site - 26 JANUARY 2022

In coordination with the Quezon City LGU, the University was granted a special schedule for booster shots and first-dose vaccines on Wednesday, 26 January 2022. The vaccination site will be at the Ateneo Grade School Cafeteria.

Available vaccines are Sinovac and AstraZeneca only. For first-dose vaccinees, you may indicate your preferred vaccine (subject to availability) on the sign-up form. For booster shots, the vaccine administrators of the QC LGU will determine which of these vaccines will be administered, based on the first two doses that you received.

This initiative is open to all employees, students, university affiliates, and their respective household members, 18 years old and above, who have completed their first two doses no later than 25 October 2021, regardless of whether they availed it from QC or elsewhere. First-time vaccinees who are non-QC residents may also avail of this vaccination program.

Please take note of the requirements indicated in the form that will be needed when you proceed to the Ateneo Grade School on Wednesday, 26 January 2022. The Campus Safety and Mobility Office (CSMO) shall send a confirmation email upon receipt of your registration on a first-come, first-served basis.

Please wait for your email notification of approval before heading to the campus.

In case there will be an overflow of registrants, we will endeavor to request QC LGU for another schedule.
